The following information was listed on the dedication plaque of USS Yamato.
This information is from the Star Trek Encyclopedia. Presumably the plaque was made for "Contagion". It was however not seen on screen in the episode.
In "Where Silence Has Lease", the dedication plaque of Nagilum's recreation of the Yamato was seen. In the remastered version of the episode the plaque was blurred in post production, because it was only a reuse of the Enterprise-D plaque.
