Ensign Phil Wallace.

Phil Wallace was an Ensign serving aboard the USS Pegasus with William T. Riker. He was killed during the testing of a Starfleet prototype Interphase cloaking device, violating the Treaty of Algeron. In 2370, Riker would recall that Wallace was a "Hell of a handball player.", and was seriously considered for a promotion, presumably to Lieutenant Junior Grade, shortly before his death (ENT: "These Are the Voyages...")
