Levinius V was an uninhabited planet. This was the homeworld of an extinct species. It was the fifth planet in the Levinius planetary system.
In 2067, the planet was invaded by neural parasites that came from Beta Portolan. The inhabitants of Levinius V were all killed in the mass insanity caused by the parasites. The inhabitants transported this parasite over to Theta Cygni XII. (TOS: "Operation -- Annihilate!")
Presumably, Levinius V was located in the Alpha Quadrant.
A deleted scene from the Star Trek: Enterprise episode "Fight or Flight" mentioned a planet called "Livinius".
