D. Okuda was a Human Starfleet officer in the 22nd century.

In 2151, she was named as an Flight Software officer on the dedication plaque of the Enterprise NX-01. (ENT: "Broken Bow", dedication plaque)

In 2154, she was named as a Flight Test crewmember on the dedication plaque of the Columbia NX-02. (ENT: "Affliction", dedication plaque).

In January of 2155, she was named as a Flight Dynamics officer on directory placards at the Fleet Operations Center, Sol Sector. Her office was in room #3505. (ENT: "Demons", directories)

Okuda was named for production staff associate Denise Okuda. There are other, later, members of the Federation's Starfleet that is also named for Okuda, indicating a possibility that "D. Okuda"'s first name is Denise, and that they might be related.
It has also been speculated that all of the 22nd century persons listed on the plaque were somehow extraordinarily long-lived, and are the same as those who were credited on the later 23rd and 24th century plaques.
