A crew personnel report (or crew report for short) is a standard report in Starfleet with the designation 15-5-01. It includes details on crewmembers, such as their medical history. Governments interacting within or with Starfleet are required to provide the medical history of people who become Starfleet crewmembers.
The transfer of crew personnel reports when a Starfleet crewmember changes assignments is the responsibility of Starfleet Medical. The Doctor thought it was the agency's fault for not having a report of Kes on file to compare brain scans. (VOY: "Time and Again")
